Form will not display when multiple form fields are included in a Group

See the attached solution that shows the form not displaying when using Grouped Fields in the GroupForm page.

If you comment out lines 197 and greater in the GroupForm page the form will be displayed.

And sets only on the second time the field is touched and lost focus and helping us improve our product.
