Multi page PDF with Header and Footer

Hi,

At present i am using a version < 9 to generate my PDF and that works fine. 

Now I would like to create a multi-page PDF with header and footer on each page. The footer on each page must have page #  example Page 1/18, Page 17/18 etc

How can I do this?

Also, in the header I want to have multiple lines like

Company name - centered

Company Address - centered

Current date - left justified
