Extract PDF layout

I have a user fillable PDF from customer. I want to extract the page layout and metat data of PDF so that I can use code , see example in controller tab

and build my PDF. Reason for doing this is that when customer changes the fields of the pdf, i want to make it configurable so that I can add the new fields in web.config and my code can loop through and generate the pdf with data filled in.

I want to use the customer pdf so that I can keep the same look and feel.

It is not possible to extract the page layout of Pdf form fields, whereas we can able to extract only the contents, can you please be more specific in your requirement by providing the input file, sample or the screenshot regarding this. It will be helpful for us to provide you solution better.Please check the sample in the below link and let me know if there is any concerns.
