Mail Merge Issue

We are planning use syncfusion product in our project but i couldnt get the expected result.I am attaching the template file and the file with expected result.Can you provide me a sample with a similar template.Please consider this as very urgent.

Hi Rajendran,
Thanks for the response.But still i haven't got the desired result.Now the value is not getting merged.I am getting a blank page with all labels but values are getting filled.Can you please provide me an updated template file from the file that i have mailed you without changing the mergefield names.
