Page 2 of Merged PDF Documents Not Displaying

I'm writing a faxing program for a client that combines files (Word, Excel, PDF, TIF) into a PDF document and sends to a fax server. The included attachment is a striped-down sample of a cover page and company document. Acrobat Reader displays both pages just fine; your viewer program does not display any content on page 2.
