If I hard code the data source of the multiselect, as I click an item, it is immediately shown in the multi select drop down as selected.
However if the data source of the multi select is loaded after render (via an API call etc) the selected items do not show until I have lost focus of the multi select. Once refocusing the items appear as clicked as expected.

And, when validating the attached sample, the reported issue is not replicated from our end. if still you have faced the same issue, then share the exact issue replication procedure. these details will help us to provide an exact solution as earlier as possible.

Is loading a DataSource without using the SfDataManager not supported then?
I can reproduce on that solution - Please see video of the issue. You can see on the dynamically loaded data, the items don't appear as they are clicked, but they do in the hard coded one.
