Currently
we do not have support for adding custom form fields in the form Designer
toolbar. But you can customize the existing form fields by enable and disabling
it in the UI level using API and for your reference we have provided a simple
sample and UG documentation below

In
PdfViewer we have implemented support only for the existing form field and we
think if you add custom form field it does not meet your requirement and let me
know what kind of field is need to be added and it will help us to provide
solution on it and let me know the
