Shapes annotation do not work on repeated load in modal window

Steps to reproduce:

  1. Click on View PDF
  2. Click on Add or edit annotations
  3. Click on Add shapes. 
  4. Shape is successfully added
  5. Close modal window.
  6. Repeat steps 2-4.
  7. It is not possible to add shape.
  8. After reloading page in browser adding shapes works.
I tested it in Opera and Firefox and it does not work in both. Project is attached.
(I had to delete pdfium.dll, packages, scripts, Content folder from attached zip file so it passes allowed upload size.)
